If you apply from abroad, you must submit the original “Foreigner Physical Examination Form” within the validity period. If you are an applicant already staying in China, you must submit the original “Certificate of Verification(For Physical Examination Record for Foreigner or Overseas Chinese” (including the Physical Examination Report) within the validity period.

Tips:
For applicants who are in China: If you are an employee, you still need to provide employment separation certificate and your current residence permit.
